01 — Konnect Call
I offer a 15-minute free consultation to discuss your case and provide some high-level insights into the issues and possible pathways available to you.
Cost: Free
02 — Konnect Contact
If after the consultation, you would like me to look at your files, go in-depth into your personal needs, I will send out a form to be filled out by the patient or the legal guardian. Once received, I will review and set up a 45 minute call to discuss the case and provide specific recommendations in writing.
Cost: $75
03 — Konnect Plan Management
If your case requires my oversight and personal attention and ability to connect the patient with one of our consultants, this will be a monthly fee to reach desired goals. I will become your case manager and assist with every aspect of your journey and work with the family and patient to create a care plan to reach the highest level of healing as possible. For this tier, the patient must be in agreement with the plan and changes to the plan will always be discussed as a team with the patient.
Cost: based on individual case and needs assessment.
Cost: $125/ Hr or base rate of $1000/month- minimum 3 months
